Ayushman bharat योजना कार्ड को downlaod करने के लिए आपको पहले एक account बना होगा. अगर आपका पहले से ही account बना है तो आप लॉग इन कर ले. नया account बनाने के लिए आपको Sign Up पर click कर दे.

Sign Up पर click करते ही आपके सामने यह page आ जायेगा. इस page में आपको अपना आधार कार्ड नंबर को डालना है. और next पर click कर दे.

next पर click करते ही आपके मोबाइल नंबर पर otp आएगा. यह otp आपके आधार कार्ड से लिंक नंबर पर आएगा. आपको उस otp को सबमिट कर दे.

अब आपको पिन सेट करना है. अपनी मर्जी से 6 अंको का पिन सेट कर सकते है. पिन सेट करने के बाद done पर click कर दे.

Health ID card क्या है?

यह health id card सरकार  के new scheme one nation one health card के अंतरगत बनाया जायेगा. इस योजना को 15 अगस्त 2020 को हमारे देश के प्रधानमंत्री ने लाल किले पर तिरंगा को फहराते हुए भाषण में इस योजना की घोषणा की और एक नए शुरू new scheme one nation one health card की शुरूआत की. इस योजना के अंतर्गत आपको बहुत सारे लाभ मिलता है.

हेल्थ आईडी कार्ड

health ID card सरकार के नए योजना के अंतर्गत बनाए जाने वाला एक digital card है. जिसमे आपका जन्म से लेकर मिर्तुयु तक का सभी medical treatment record save रहेगा. जिसमे आपने जन्म से क्या-क्या टेस्ट करवाए  है आपको कोन- कोन सी दवाई गई है. यह सभी जानकारी आपके हेल्थ आईडी कार्ड में save रहेंगे. जब भी आप कही इलाज के लिए जाते है तो आपको किसी भी तरह का पेपर नही ले जाना होगा. बस आपको  अपना हेल्थ आईडी कार्ड ले जाना होगा जिससे Hospital वाले आपका डाटा आपके हेल्थ आईडी कार्ड ले लेंगे और इसे doctors को आपका इलाज करने में आसानी होगी. और doctors आपको जो भी treatment देंगे वह सभी treatment record आपके हेल्थ आईडी कार्ड में save हो जायेंगे.

इस योजना में आवेदन करने के लिए आपको पहले https://healthid.ndhm.gov.in/ पर जाना होगा.

अब आपको Create your Health ID Now पर click करना है.

Continue पर click करे.

अब अगर आपके आधार कार्ड से मोबाइल नंबर लिंक है तो आप Generate Via Aadhar पर click करे अगर आपके आधार कार्ड से आप का मोबाइल नंबर लिंक नही है तो आप दुसरे आप्शन पर click करे.

जैसे ही आप I don’t have Aadhaar / I don’t want to use my Aadhaar for creating Health ID. पर click करेंगे तो आपके सामने यह page आएगा जिसमे आपको Generate Via Mobile पर click करना है.

अब आपको अपना मोबाइल नंबर डालना है. और I agree पर click करके Submit पर click कर देना है.

अब आपके मोबाइल नंबर पर otp भेजा गया है जिसे आपको इस बॉक्स में डाल देना है और Submit पर click कर देना है.

Submit पर click करते ही आपके सामने यह फॉर्म ओपन होगा जिसमे आपको सभी details को भर देना है. आपको email id की तरह आपको एक unique ID बना लेना है और Submit पर click कर दे.

Submit पर click करते ही आपके सामने Congratulations का message आएगा. अब आपको नही आ जाना है.

निचे आने के बाद आपको आपकी सारी details दिख जाएगी जिसमे आपका कार्ड नंबर और आपकी basic details होगी. आप देखेगे की आपके health card पर आपकी फोटो नही है तो आप इसके लिए Edit Profile पर click कर दे.

और अपना फोटो सेट कर ले.

फोटो सेट करने के बाद आपको निचे आजाना है और Submit पर click कर देना है.

अब आपके id कार्ड पर आपकी फोटो सेट हो चूका है. अब आप Download Health ID Card पर click करके Download कर लेना है और इसका प्रिंट कर लेना है. जिसके बाद आप कभी भी इस Health ID Card को किसी भी Hospital में उपयोग कर सकते है.

नोट: अभी इस योजना को छोटे राज्यों में शुरू किया गया है.और अभी यह बिलकुल मुफ्त है. इसका आवेदन कोई भी कर सकता है.

After which you have to go down again and fill your email id and mobile number, and the captcha which is visible in front of you has to be filled in the captcha box. And click on continue.

As you can see in this photo on the right side, you will also get such a message. Now if your mobile number is linked with your Aadhar card, then you click on Generate Via Aadhar, if your mobile number is not linked with your Aadhar card, then you click on the second option.

On clicking submit, this form will open in front of you, in which you have to fill all the details. You have to create a unique ID like email id and click on submit.

As soon as you click on submit, you will get a message of congratulations. You don’t have to come now.

After coming down, you will see all your details, which will contain your card number and your basic details. You will see that you do not have your photo on your health card, then you can click on Edit Profile for this.

And set your photo.

After setting the photo, you have to go down and click on submit.

Now your photo has been set on your id card. Now you have to download it by clicking on Download Health ID Card and print it. After which you can use this Health ID Card anytime in any hospital.
